Completing this application does not automatically qualify you to obtain a free mammogram. A representative from Soroptimist International of Stuart will contact you for a brief telephone interview and to obtain additional information. You may be required to provide proof of residency and financial need.

You must meet all of the following criteria in order to qualify for a free mammogram I am a resident of Martin County I do not have private health insurance, medicaid or medicare I am not able to pay I have a physician's order for a screening or diagnostic mammogram I have not had a mammogram within the last 12 months No history or current breast problems such as lumps, pain, nipple discharge.
